Combined graft of the pancreas and kidney - implantation of grafts transplants (enteric drainage). The total pancreas transplantation is generally done in treatment of diabetes type 1, when the patient does not respond to insulinotherapy, that he presents severe complications or that he is in terminal renal insufficiency. The intervention is done in three steps : sampling of the transplant (see. image 1074807), reconstruction of the transplant vascularization (see. image 1074907) and transplantation (here and image 2862808). The pancreas of the donor has been entirely sampled with the duodenal frame, then the vascularization of the transplant was restored. The pancreas is transplanted at the level of the right iliac vessels of the receiver (systemic venous drainage) and the pancreatic secretions are drained into the jejunum (enteric drainage by duodeno-jejunal anastomose ). This procedure is named Roux-en-Y pancreatico-enterostomy.
