Initially, the religious order of the Tertiaries was established mainly in Liguria, on the Riviera and inland, but within a few years it spread to Piedmont and Lombardy. The irradiation of their service is originally a consequence of the appeal of mayors or Presidents of the Holy Works who ask the religious for a nursing service and help to the sick during the frequent epidemics, not secondarily chosen also because it is inexpensive. It was in fact the individual municipalities that at the end of the century, in front of a few and dislocated civic hospitals, provided health care for their citizens, especially in case of epidemics of cholera, typhus, smallpox, viral flu etc. and natural disasters such as earthquakes.
